Start with a defined trigger

Gather permitted logs, affected-resource references, recent approved changes and the service runbook. Limit access to the incident's scope.

Perform the bounded task

Summarize the observed symptoms, list supporting evidence and propose diagnostic questions rather than executing changes.

Review before taking action

An engineer validates the analysis and selects any remediation. Production actions require the normal change or incident authority.

Deliver and record the result

Attach the reviewed findings and chosen next steps to the incident record with evidence references.

Build an improvement loop

Measure unsupported diagnoses, useful evidence retrieval, investigation time and operator correction effort.

Keep the input, relevant context, review decision and final result connected. When a reviewer corrects something, use that evidence to improve the instructions or integration, then validate the change against representative cases.
