Answer the questions
that shape the build.

Who made the decision? Which constraints mattered most? What future change would justify revisiting it?

Do not manufacture consensus or claim that an option was evaluated without evidence. Record uncertainty and known limitations.

We use these answers to agree on integration boundaries, responsible reviewers, access controls and a definition of done. The proposal identifies dependencies and what is included before implementation begins.

Pilot one recent decision and ask participants to verify the alternatives and rationale.

Measure the result

Measure omitted constraints, invented reasoning, unresolved disagreement and time to a useful record.

Prepare for everyday operation

Store the accepted decision with the project documentation and link affected work.

Document the owner, support path and recovery procedure. Define how the workflow behaves when information is incomplete, access fails or a reviewer declines the proposed result. Agree on what needs fresh validation after a model or integration change.
